Smithfield is located at 38°23′8″N 85°15′21″W / 38.38556, -85.25583 (38.385521, -85.255940).
According to the United States Census Bureau, the city has a total area of 0.1 square miles (0.3 km²), all of it land.
As of the census of 2000, there were 102 people, 43 households, and 26 families residing in the city. The population density was 763.1 people per square mile (302.9/km²). There were 47 housing units at an average density of 351.6/sq mi (139.6/km²). The racial makeup of the city was 99.02% White and 0.98% Native American.
There were 43 households out of which 30.2% had children under the age of 18 living with them, 51.2% were married couples living together, 4.7% had a female householder with no husband present, and 39.5% were non-families. 37.2% of all households were made up of individuals and 14.0% had someone living alone who was 65 years of age or older. The average household size was 2.37 and the average family size was 3.19.
